🧑‍💻 Key Responsibilities

HCC Coders

• Perform Hierarchical Condition Category (HCC) coding based on medical records
• Assign accurate diagnosis codes as per ICD-10-CM guidelines
• Ensure compliance with CMS risk adjustment models
• Maintain coding accuracy and productivity targets
• Review clinical documentation for completeness

HCC QA Coders

• Conduct quality audits of HCC coded charts
• Identify coding discrepancies and provide feedback
• Ensure adherence to coding standards and compliance policies
• Train and mentor coders to improve accuracy
• Maintain audit reports and quality metrics

🎓 Qualifications & Eligibility

• Experience:
  • HCC Coders: 1–4 years
  • HCC QA Coders: 2+ years (QA experience required)
• Certification: Mandatory – CRC / CPC / CCS
• Strong knowledge of:
  • ICD-10-CM coding guidelines
  • Risk Adjustment & HCC coding
  • Medical terminology and anatomy
